Description
The QAPI Coordinator will develop, implement, and maintain Quality Assurance and Performance Improvement programs to ensure compliance with regulatory requirements, enhance patient care, and achieve quality outcomes. This role involves data analysis, identifying improvement opportunities, and supporting clinical and operational teams in implementing best practices. The ideal candidate will have strong knowledge of home health and hospice regulations, as well as a passion for continuous improvement in patient care quality and safety.
Key Responsibilities:
- Develop and Manage QAPI Program: Create, implement, and oversee a comprehensive QAPI program that meets state and federal guidelines, including CMS requirements for Home Health and Hospice.
- Performance Improvement Projects (PIPs): Identify areas for improvement and initiate Performance Improvement Projects to address quality and compliance gaps.
- Data Collection and Analysis: Gather, analyze, and interpret quality and performance data. Use data to track progress, outcomes, and identify trends in patient care.
- Compliance: Ensure the agency’s practices align with all regulatory standards, including federal, state, and local regulations. Maintain a comprehensive understanding of CMS QAPI regulations.
- Audit and Review Processes: Conduct regular audits and chart reviews to assess the quality of care and identify areas for improvement.
- Education and Training: Develop and deliver training sessions on quality standards and compliance for staff. Support clinical teams in understanding QAPI standards.
- Documentation and Reporting: Maintain detailed and accurate records of quality data, incident reports, and corrective actions. Prepare and present reports for QAPI committee meetings.
- Interdepartmental Collaboration: Work closely with clinical teams, administration, and management to foster a culture of quality and compliance within the agency.
- Patient Satisfaction: Monitor patient feedback and satisfaction data, addressing areas of concern through QAPI initiatives.
- Root Cause Analysis (RCA): Conduct RCA for adverse events, identify underlying causes, and collaborate on corrective actions to prevent recurrence.
Requirements
1. 3-5 years U.S. Home health and Hospice experience is preferred
2. Excellent understanding of performance improvement, quality assessment, and utilization management.
3. Is self-directed and able to work with minimal supervision.
4. Has excellent coordination and communication skills.
