Description

  • We are currently seeking a compassionate and detail-oriented individual to join our team as a Home Health Intake Specialist. In this crucial role, you will serve as the initial point of contact for patients and families seeking hospice care services. Your primary responsibility will be to coordinate the intake process, ensuring a smooth transition for patients into our hospice program.
  • Responsibilities:
  • Check patient details of patients referred and request medical records from the hospital where the patient is admitted.
  • Assess patient eligibility for hospice care, and complete the admission process.
  • Serves as a point of contact for patients and their families, providing information about hospice services, answering questions, and addressing concerns. Offers emotional support and guidance throughout the intake process.
  • Verifies insurance coverage for hospice services, assists with obtaining authorization, and explains the financial aspects of hospice care to patients and families.
  • Collaborates with the interdisciplinary team, which may include nurses, doctors, social workers, and chaplains, to develop a comprehensive care plan for each patient and ensure that the team is informed about the patient's admission and any specific needs.
  • Ordering medications of patients, coordinating with hospice medical director/doctor on processing medication reconciliation or changes on medication that patient is taking.
  • Coordinate with the facility where the patient is residing to inform or send them changes in the patient's plan of care.

Requirements

  • Experience handling calls is required
  • Billing experience is preferred
  • Strong organizational skills and attention to detail, with the ability to manage multiple tasks and prioritize effectively.
  • Ability to work collaboratively within a team and effectively communicate with healthcare professionals and referral sources.

Compensation:

  • Monthly Rate: P18,000

Work schedule:

  • Monday to Friday, 8:00 AM - 5:00 PM PST
